Benefits Enrollment - New Hires

Employees become eligible for benefits on the first of the month following their 60 days of employment. The date they are eligible is tracked in the X drive under X:/HR/HR New Hires/Terms Leaves 2019-2022.   

Three weeks prior to the employee's enrollment eligibility date, the following email is sent.  Copy and paste the below template into the email and input the effective date for the employee's eligibility.

Good Afternoon,              

It is time to complete your benefit enrollment! Please log in to your ADP account and go to Myself > Enrollments (under benefits) – see below. You should see a link after you click on Enrollments that says ‘New Hire Enrollments’. Your benefits are effective Effective Date and your enrollments must be completed at least one week prior to your effective date. Attached is the Surya Benefits Booklet that explains all of the benefit options, you will be able to see the costs as you go through the enrollment process.  

All accompanying benefits documentation can be found on the ADP homepage including the Summary of Benefits & Coverage (SBCs) and Certificates of Coverage (COCs) for the 4 medical plans, Prescription Essential Drug List, Dental Summary and Vision Summary. All required notices are also listed on the ADP homepage.
